Adult Services Navigation & Application Guidance

Many families discover too late that accessing adult services requires early preparation, documentation, and strategic timing.

We provide guidance to help families understand available systems and navigate the process of preparing for adult services.

This may include support with:

• Understanding eligibility for services such as DDS and related programs

• Organizing documentation needed for applications

• Creating timelines for transition milestones and service access

• Identifying community programs, vocational supports, and postsecondary options

• Providing personalized resource guides and practical checklists

Transition Planning & Independence Development

Preparing for adulthood is one of the most important transitions a family will navigate. Yet many families find that planning for life beyond high school is fragmented, confusing, or begins far too late.

We provide transition-focused consultation and skill development for adolescents and young adults, helping families move from uncertainty to a clear, strategic path forward.

Our work integrates planning, system navigation, and real-world skill development so that today’s supports meaningfully prepare individuals for adulthood.

Applied Independence Training (Ages 12–22+)

​Planning for adulthood is not only about services, it is also about building the practical skills needed for independence.

We offer community-based transition support that allows adolescents and young adults to practice real-world skills in authentic environments.

• Community navigation and safety

• Grocery shopping and meal preparation

• Budgeting and money management

• Transportation and routine planning

• Social communication and confidence in public settings

• Executive functioning for daily living

Skill development takes place in real-life settings where individuals can practice, build confidence, and generalize new abilities.
